Certain procedures are deemed "cosmetic" by Insurance Companies and will require payment at the time of your visit. Your physician will inform you of this if necessary.
Financial responsibility for services rendered ultimately rests with you, regardless of insurance coverage. Your policy is a contract between you and your insurance company. Co-payments and non-covered services are payable at the time of service.

Please Note: our office requires that all New Patients have their own insurance card, photo ID, and referral (if applicable) at time of visit. If you do not have all necessary information, your appointment will be rescheduled.
HMO programs require a referral from the primary physician. An appropriate referral must be provided at the time of service rendered.
INSURANCES OUR OFFICE DOES NOT ACCEPT:
Our office policy regarding medication refills stands as follows: for oral medication, we will refill prescriptions up to six months after your appointment; for topical medications, we will refill prescriptions up to one year after your appointment. If you need a prescription refilled between appointments, please allow 24 to 48 hours for the office to contact your pharmacy.
